Description
of project—What is the project? What is the
nature of your business? (i.e., service, retail, wholesale, non-profit,
etc.)

Who is going to be involved in
the project? Who will be the project manager? Previous experience
with managing project of this kind? Who is key management? (enclose
resume)

What is the plan for implementation
of the project? What is the expected timeline for implementation
of each of the activities or steps involved? When will the project
be completed?

Attach a line item budget,
including funds from other sources. Specifically identify what Oregon Investment
Board monies will fund vs. other funding sources. Provide commitments
from other funding sources.

Funding schedule. When
will Oregon Investment Board funds be needed, and for which activities?

Activities still required before
the project can proceed? Other unique characteristics of the project?

If
this is a loan request from an established business, please provide
a current Balance Sheet
and the last three years income tax returns. If this is a loan
request from an emerging business please provide a pro forma balance
sheet and three years projection of profit and loss.

Should this project require any licenses,
permits, etc. from any governmental agency, or any
other agency, copies of these documents should be made available as
part of the applicant’s application. Copies may accompany the
application, but originals must be made available at the time the
Columbia River Gorge Commissions application is submitted for certification
which occurs at the time the Oregon Investment Board gives preliminary
approval for the project.

Where, specifically, will the project
be located and what portion, by percentage, of benefit
will accrue to the National Scenic Area and how was the percentage
calculated? What will the leverage be (i.e. what is the percentage
of this request to the total project cost)?
NOTE: If your project is in an unincorporated area outside
of the City of Hood River, Cascade Locks, Mosier or The Dalles, please
refer to the specific requirements of the Gorge Commission, Section
3, Columbia River Gorge

Should the project be approved, what criteria can be used to measure
successful performance? What will you measure the
success of this project? (e.g., create four family wage jobs;
bring a new product to market that will add value to locally produced
agriculture products, etc.)
